If you or your organisation were successful in receiving a JMA Award, we very much hope that the funds have made a positive impact, and that you’ve realised the aims set-out in your application.
Around six months following your Award, we would really appreciate your completion of the End of Award Form found below. Monitoring how awards have been used and sharing the stories of applicants with our supporters are crucial to our work as a charity. We’re also keen to identify any positives or short-comings applicants might have encountered in dealing with the JMA, so we can continue to develop and improve.
We’d appreciate if you could fill in the form and return it to us. We’ve tried to keep the form as simple as possible, but if you have any questions or concerns, please let us know by contacting us.
The form can be completed in online format. This online version is also complete with an File Uploads folder at the end. Feel free to use this to send us any extra information or materials you think might help us to get a sense of the difference your JMA award has made, such as testimonies or photos.
The form is also available in PDF format. This can be easily downloaded and printed for completion by hand if this is your preference. For these, please either scan and email your completed form to firstname.lastname@example.org, or post to the address on our Contact Us page.
